Coordination of Care

Helping families who receive services from multiple providers access the services they need while reducing duplication.

Some of the core work associated with a systemic approach to Coordination of Care:

  • Developing a system-wide process for coordinating care when families are accessing services across agencies and/or service sectors
  • Assuring families are accessing the services they want and need
  • Eliminating unnecessary and inappropriate duplication of services
  • Enhancing families’ capacity to coordinate their own care
  • Monitoring the coordination of care process through ongoing collection of data and stories to:
    • Evaluate the impact of the coordination of care process
    • Identify opportunities to improve the process and outcomes
  • Continually improving the coordination of care process
